CY 2013 - This rate is based on a low number of patients. Consumers should use many sources in making choices for health care services. For 2013, the data for outcome measures for First Choice Community Health, like Blood Pressure Control, Blood Sugar Control and Cholesterol Control, were collected using medical record reviews with small sample sizes. The sample sizes were smaller than in previous years and may impact the reliability of the rates. Since this is based on limited identifiable patients, consumers should use many sources in making choices for health care services. In addition, this decrease in sample size is the reason clinics were unable to report these outcomes measures.
